The CARE-D-Foot-Nav study is designed to see if a special program can help heal diabetic foot ulcers (DFUs) better than usual care. DFUs are serious wounds that occur in people with diabetes, sometimes leading to amputations. This study involves 270 patients who will either get regular care or join the CARE-D-Foot-Nav program for 20 weeks. If you join the program, a trained helper called a patient navigator will support you weekly. They'll help with diabetes education, arranging transport, and connecting you to medical and social services. This support aims to improve healing and make care more accessible, especially for underserved communities.

Key Points:

  • The study lasts for 20 weeks with weekly support from a patient navigator.
  • Participants will receive help with diabetes education and accessing resources.
  • The goal is to improve healing and reduce the need for amputations.
