Phase N/A

This study aims to help people with advanced Chronic Kidney Disease (CKD), a condition where the kidneys don't work well over time. The project uses community-engaged processes to create a system called Clinical Decision Support (CDS) to improve care. It also includes Kidney Health Coaching (KHC), where trained coaches help patients manage their kidney health.

The goal is to see if KHC can delay the need for Kidney Replacement Therapy (KRT) or central venous catheter use. A catheter is a tube inserted into the body to remove or give fluids. The study will last 6 months, and participants will receive coaching during this time. Patients will be contacted by phone to give their consent and complete an assessment.

  • Participation requires 6 months of kidney health coaching.
  • Eligible participants are African American or Black with specific health criteria.
  • Participants cannot be on dialysis or in another clinical study.

The study is looking to understand how coaching can help patients manage their CKD better and delay serious treatments. If you meet the criteria, you may be invited to join and help improve kidney care for others.
