This study is about helping doctors, patients with memory problems, and their families talk better about healthcare goals. Memory problems, like Alzheimer's, can make it hard for people to think and remember things. The study uses a tool called the "Jumpstart Guide," which is a simple guide with tips for doctors. It helps them have good discussions with patients about their healthcare wishes.

Doctors will get the Jumpstart Guide before the patient's clinic visit. Researchers want to see if this guide helps more patients and families talk about healthcare goals. After the doctor's visit, patients and families will complete surveys to see how well the guide worked.

  • The study involves several visits over 12 months.
  • Participants might be asked to fill out surveys and take part in interviews.
  • There is no specific mention of compensation for participating in this study.

If you are 55 or older, have memory problems, and visit a UW Medicine clinic, you might qualify to join this study. Family members and doctors can participate too if they meet certain criteria. This study aims to improve how healthcare goals are discussed, making sure patients' wishes are understood and followed.
