Pediatric-Adult Care Transition Program for Sickle Cell Disease

This study focuses on helping teenagers with sickle cell disease (SCD) as they move from child to adult healthcare. Sickle cell disease is a condition where blood cells are abnormal, causing pain and other serious health issues. The study aims to make this transition smoother by involving both pediatric (child) and adult health services. The program uses a special approach called the biopsychosocial model, which looks at not just the physical health but also the mental and social aspects of the patient’s life. Researchers want to see if this program reduces hospital visits over two years. They will study 196 teens who are at least 16 years old.
  • Study lasts 24 months, focusing on reducing hospital visits.
  • Involves 196 patients, all aged 16 or older with SCD.
  • Aims to improve quality of life and healthcare use.
The goal is to improve health and family relations, helping teens manage their disease better as they grow up.
